I&M Society Best Dissertation Award

Please have the following materials available before completing this nomination form. Uploaded materials should be in PDF:

1. Contact information for the candidate and the nominating sponsor (name, institution, mail and e-mail addresses, IEEE member number)

2. Abstract of dissertation (maximum 1000 words to be used as highlight article in the IMS Magazine if awarded)

3. Signed copyright form for the abstract (IEEE Copyright and Consent Form)

4. Copy of dissertation in a pdf file

5. Letter of recommendation from the nominating sponsor including a mandatory statement of the importance of the work to the I&M field

6. Up to two additional letters of recommendation

7. Unofficial transcript of the nominee (Unofficial transcript example)

8. CV of the nominee

9. Biographical and personal statement written by the nominee (up to 2 pages outlining career and personal goals, education, research interests and other relevant information of the student).
